Production Agreement Forms
Overview
All student productions must formally notify cast and crew that our production insurance policy does not provide worker’s compensation, medical injury, or auto coverage to all students and unregistered volunteers.
Agreement forms are required to be completed by all cast and crew members for each student production. Student Producers are expected to have these forms completed prior to start of principal photography and should not permit any cast or crew members to participate on their productions until they have signed the form. Blank copies should be available on-set for any last minute day players that show up to help out.
Students are required to turn in all signed copies of this form to their instructor-of-record for record keeping at the completion of their production.
Below are summaries of who or what each agreement form is for.
Students
Any crew members, who are currently enrolled at UCF and is in good standing as an undergraduate or graduate student, are required to sign a Student Agreement Form.
Unregistered Volunteers
Any crew members, who is NOT currently enrolled as a student, registered as a volunteer, or employed as a staff or faculty member at UCF, are required to fill out a Volunteer Agreement Form.
The most common example of this are friends and/or family members of the student filmmaker helping out with production.
Crew members who are being compensate by a student Producer to work on their production, are also required to fill out the Volunteer Agreement Form. They are not considerd employees of the University of Central Florida Board of Trustees and thus are NOT covered by the university’s worker’s compensation policy.
Registered Volunteers
Reserved for non-student professionals who volunteer their time to provide workshops or participate on student productions without the expectation of compensation. This process provides coverage of the university’s Worker’s Compensation policy if specific criteria is met and must be completed prior to any registered volunteer work begins.

Staff & Faculty
Any crew member who is currently employed by the University of Central Florida Board of Trustees is NOT required to fill out an agreement form. This does not include student employees unless they are participating on the production as part of their assigned job duties.
Location Agreement Form
Students should get the property owners, of all the third-party locations they use in their productions, to sign a Location Agreement Form.
The location agreement not only helps to protect UCF and NSCM, but it also benefits the student by clarifying many of the details of the arrangement between the property owner and the student production. The instructor-of-record or thesis advisor may not allow production to begin until all signed copies of this document are obtained.
In some cases, in addition to the Location Agreement Form, students may also need to obtain a permit when using or impacting public property. Please discuss each location with your Instructor-of-Record to help determine what is approriate.
Students are expected to provide a copy of the final shooting script to the property owner and have agreed to proceed with permission to utilize their property for the student production.
Location Agreement Forms are not required from locations owned by UCF, but student productions are required to obtain the necessary authorizations to use UCF property or facilities.
