Certificate of Insurance (COI)

Overview

Certificates of Insurance (COI) is an official document that provides a summarization of coverage, brokerage contact information, and discloses the underwriter that bonded the policy. Rental companies and property owners often request proof of insurance in the form of an insurance certificate. COI’s are available for issuance to third parties as verification that a sanctioned production, and related activities, are covered by the school’s Production Insurance Policy.

COI’s are only available to Full-Service Productions that have submited a Risk Management Assessment form and have been approved by the Head of Film Production Services. Turnkey Productions must elevate to a Full-Service Production to request a COI.

COI’s are not available for student Media Assignment.

Students who request the issuance of a certificate for personal use, or for any other purpose then to support a sanctioned production for their course, study, or thesis prompted creative project, is misuse and can be considered insurance fraud.

A minimum turnaround of three-business days is required when requesting certificates.

How to Request a COI

For Locations

  1. Download a Certificate of Insurance Request Form from the Policies and Procedures section.
  2. Please clearly indicate which scenes will take place at this location by highlighting them on a PDF copy of the entire script or outline for your project.
  3. EMAIL this form and script / outline to the Insurance Coordinator AND Head of Production Services. This form must be completely filled our, with instructor-of-record signature, or it will be rejected without review.
  4. Student will be emailed the certificate upon approval or notified of why issuance cannot be made.

It is the student’s responsibility to deliver the issued certificate to the individual/company requesting issuance.
